LUPAs are just one of the contractual adjustments that currently make up the Medicare-certified home health prospective payment system. Home health agencies have had a five-visit LUPA threshold for some time now with four visits or less per episode resulting in a low-utilization payment adjustment. There are many different factors that can cause a LUPA to occur. An expected LUPA occurs when an agency admits and understands that a particular patient will not require enough visits to exceed the LUPA threshold.

Home Health Lupa Definition
The PDGM will allow HHAs to designate one principal diagnosis and up to 24 secondary diagnoses. This is in contrast to the five secondary diagnoses currently allowed on the OASIS assessment. According to CMS, the comorbidity adjustment can increase payment by up to 20%. Because clinicians can document a greater number of secondary diagnoses under PDGM, CMS assumes that more 30-day periods will receive a comorbidity adjustment than if only OASIS codes were used for payment. Thorough assessments, ongoing reassessments and accurate documentation are critical to this assumption. It’s possible that the first 30-day care plan with the additional visits might produce better outcomes and the second 30-day period may not be needed and a LUPA can be avoided.

They have a patient, dropped the RAP, but don't know what the LUPA threshold is when developing the Plan of Care . If the agency knows what the LUPA threshold is, they can plan right up front. For example, a patient could have a LUPA threshold of 4 in the first 30 days and 2 in the second 30 days. Agencies now know that you have to have more than 4 visits to avoid what we call LUPA land in the first 30 days.
One of the most significant changes CMS has made in the new payment model is the threshold for a Low Utilization Payment Adjustment, or LUPA. Under PDGM, billing occurs on a 30-day payment period instead of the current 60-day period. The shorter period for payments means the threshold for a reaching a LUPA also changes, but there is more to the change than simply cutting the number of acceptable visits in half.
Effective management of LUPA episodes has always been a challenge and yes, in the PDGM world, it just got trickier.
